Book Your Vascular Visit
The fastest way to book is to call 904.493.3333. You can also send an appointment request through FCCI's contact page.
Call to book
- Jacksonville and most offices: 904.493.3333
- Orange Park: 904.644.0092
- Palatka: 386.325.2836
The Jacksonville line handles the Southside, Northside, Westside, Mandarin/Bartram and South Georgia offices. Orange Park and Palatka have their own lines. If you are not sure which office is best, call the main number and the team will help you choose. Have your insurance card nearby when you call.
If you are coming in about booking your first visit, bring a short symptom timeline, a complete medicine and supplement list, your insurance card and a photo ID. Those details save time in the exam room.
Ready to talk about booking your first visit? The main line is 904.493.3333, Orange Park is 904.644.0092 and Palatka is 386.325.2836. Save personal health details for the phone call or your visit rather than an online form.
Request online
Prefer to write? Use the request form at firstcoastcardio.com/contact-us and the office will reach out. Please share only your name and how to reach you. Do not put health details in an online form; save them for your visit or the phone call.
After you send the form, watch for a call back from the FCCI team to set your appointment time. If you need to be seen soon, calling is faster.
Wondering if insurance covers booking your first visit? FCCI works with Medicare, Tricare, Blue Cross Blue Shield, Aetna, Cigna, United Healthcare, Humana (some exceptions apply) and a long list of others, and staff can verify your plan by phone.
Results about booking your first visit are reviewed by your doctor, who then builds a plan around you. That can mean a walking routine and medicine, or a procedure if blood flow needs help.
What to bring
- Insurance card and photo ID
- A list of your medicines and doses
- Notes on your symptoms: when they start and what helps
- Names of your other doctors, including your nephrologist if you are on dialysis
Mention anything that affects booking your first visit: diabetes, kidney disease, smoking, earlier heart or leg procedures, or a family history of aneurysm or stroke. A family member can come along to help listen.
Ahead of any procedure for booking your first visit, follow the meal and medication instructions closely. Patients with diabetes receive tailored advice about their medicines.
Dr. David Swain, a podiatric surgeon, helps with booking your first visit. His focus is wound care and limb salvage, he holds board certification in Limb Preservation and Salvage, and he volunteers with A Leg Up: The Jacksonville Amputation Support Group.
Urgent symptoms
If a leg or foot suddenly turns cold, pale, numb or very painful, or you have sudden weakness on one side, trouble speaking or vision loss, call 911 now.
Sudden symptoms are emergencies. A cold, pale or numb leg, sudden one-sided weakness, trouble speaking, chest pain or sudden severe belly or back pain all need 911, not an appointment.
One thing worth knowing about booking your first visit: FCCI pioneered outpatient catheterization in this region, an approach it says trims out-of-pocket costs and feels more relaxed than a hospital. The Jacksonville lab dates to 2007 and the Palatka lab to 2016, both accredited by the Accreditation Association for Ambulatory Health Care.
Testing for booking your first visit happens at FCCI's accredited Non-Invasive Vascular Lab, the only Jacksonville lab devoted solely to vascular testing. It studies leg, neck, kidney and belly arteries, veins for clots and valve trouble, and dialysis fistulas and grafts.
